On the first login page, click on the Continue Login button.
On the next login page, enter your unity ID and password and click Login.
After successful login, you should see a page that looks like following:
Click on Instructor link. You should see a menu which looks like the following:
Click on Post-Event Followup. The menu below will appear on the following page:
Click on the Followup link next to event/session name for which you want to create the report.
On the following page and in Step 1, you should see a list of the poeple who had registered to attend the event/session. The figure below shows such a listing:
Check the boxes next to the participants who attended the workshop. At this point you can also check boxes for individuals who are on waiting lists. If there are individuals who have attended the workshop but have not registered, you may add them after you submit the Followup report. Click here for instructions on adding a user to the roster.
In Step 2, enter the evaluation questions and click the Submit button. You should see a menu similar to the figure below:
