The insect you have described is a leafhopper. Leafhoppers
feeding in early spring can cause the leaves to become distorted.
In addition, egglaying can cause punctures in the leaves. Both
injuries last the lifetime of the leaf. Orthene, Sevin,
Insecticidal Soap, or Pyrethrin can kill leafhoppers, however,
most damage has already occurred by the time the leafhoppers are
noticed.
