The insect you have described is a weevil or beetle. The most
common weevil on rhododendrons is the Black Vine Weevil. It makes
notches in the leaf margin. Another beetle is the Cranberry
Rootworm which chews holes in the leaf, some of which are cresent
shaped. Both can be controlled with Sevin.
