Submitting a sample for diagnosis.
Plants submitted for diagnosis should include a stem with several leaves from the diseased portion plus one quart of soil containing some roots from the diseased plant. The top and roots should be placed in seperate containers. Plastic bags work good for this. For complete instructions go to the Ag Chemical Manual Samples can be submitted to the local Cooperative Extension Office. Samples can be submitted directly to The Plant Disease and Insect Clinic Box 7211 North Carolina State University Raleigh NC 27695-7211. This will cost more so check for current fee structure.
