Nominations more than 1 (one) page (back and front) will NOT be reviewed. Award nominations will only be accepted in straight text, Microsoft Word or StarOffice writer format. No HTML or PDF formats will be accepted. Margins must be 1-inch and information presented must be of 10-point type.

Listed below is the information to be included in the nomination for individuals for Epsilon Sigma Phi Recognition and Awards. Be sure that all information is included. Nominations that exceed 1 page (front and back) will not be considered.

Individual nominees must be current ESP members, and at least 50% of team members eligible for ESP membership must be current members.

Remember nominations are not complete and will not be considered without the submission of a photograph of the individual or group.

DESCRIPTION: Each year this recognition may be presented to one team and one individual whose innovative programming reinforces the organization's commitment to reaching diverse audiences. The program must have been implemented in the past three years.
