Objective: To recognize a 4-H Agent for exceptional capacities to manage a 4-H Program which demonstrates innovation and excellence in 4-H youth development program design, implementation and balance across program delivery modes.
Award: Five hundred dollars (pre-tax or reimbursable expenses) and plaque, awarded annually, at the Awards Banquet of the Annual meeting of the North Carolina Association of Extension 4-H Agents.
Eligibility: Any active North Carolina 4-H Agent regardless of tenure.
Nomination: Nominations may be submitted by 4-H Volunteers, 4-H Agents, County and District Extension Directors, and NCSU Faculty to the Recognition Committee of the NCAE4-HA.
Application: Application should include,
1. Letter, not to exceed one page in length citing the nominee's qualifications as they relate to program design, and implementation, focusing on innovation and excellence in 4-H program delivery methods in the specific county program.
2. One to three years of ES-237, ERS, Accomplishment Reports, and any other data drawn from exisitng 4-H youth development reporting/data bases, (ie. Use existing reports, do not create new reports).
3. Dr. Michael A. Davis Family 4-H Award for Innovation and Excellence in Program
