NCAE4-HA DALTON R. PROCTOR ACHIEVEMENT IN EXCELLENCE
OBJECTIVE: To recognize current NCAE4-HA members
with 15 or more
years of experiences and accomplishments in the 4-H
profession.
AWARD: Plaque and monetary award (Either pre-tax or reimbursable
expenses)
Eligibility requirements:
- Fifteen or more years as of December 1st in youth
development;
- Member of NCAE4-HA;
- Past recipient of DSA;
- County or area extension 4-H agents candidates.
Specific Criteria:
- Professional Contributions to NCAE4-H and affiliates
(National, District, Family and Consumer Science, Agriculture
and/or
Specialist Association).
- Program Accomplishments:
Summarize outstanding career accomplishments and clearly
show role in these areas.
- Personal Community Accomplishments:
Describe important leadership and citizenship
contribution as it relates to involvement in: community,
civic, service organizations and other important volunteer
roles.
Application should include:
*Attached Member Recognition Form;
*No more than 3 pages of narrative addressing the specific
criteria above
(professional,
program, and community accomplishments).
